Please, READ the following in it's entirety.

1. This portal is not to be used to send messages, you must call the office.  Our phone lines open at 9:00 am
Monday to Friday.

2.  If you are uploading a form, please ensure YOUR PART is complete.  Any incomplete forms will be rejected and will delay processing.

​

3.  Do not upload pictures of forms.  They do not reproduce well.  A scanned PDF is the preferred format.  Alternatively, you may drop the original off at the office.  It is the patients responsibility to provide a working copy.

4.  Please note it can take  UP TO 20 BUSINESS DAYS TO COMPLETE AND THERE MAY BE A CHARGE.

5.  Uploaded items will not be looked at in any particular time frame. If need be, contact your employer and inform them of the estimated time frame.
